For professionals

For discharge planners, case managers, guardians and attorneys.

You need the right place for someone, and you need it fast. Tell us what they need. No name required. Homes that fit answer yes or no with the open bed and the price, and you get a one-page summary for your file.

We never pay or reward anyone for sending us a client, and we never accept a request that asks us to.

Who this is for

Pick yours for a form made for your work and a printable list of the papers homes usually ask for.

Health plans: send your whole list

Long-term care plans often have several members waiting on a home at the same time. Send them all at once, no names, and get a weekly email of open beds in your network.

Send a list

How it works

  1. You send the needsCare needs, payer, area, room and who signs. No name, no date of birth, no record number.
  2. Homes that fit answerEach home says yes or no, with the open bed and the price it will charge.
  3. You get the yesesA list of homes that said yes, each with its license, its letter grade from its AHCA record and the date of that record, and a one-page summary of the homes considered, for your file. A grade sums up the state's record, not the care. Many licensed homes have a C, so read the record and ask.
  4. The family or guardian picksOnce they choose a home and agree, names go to that home, and the papers go straight to it by fax.

A note for guardians and attorneys

Many small homes work well with court-appointed guardians. Each home's page shows whether it does, and every home confirms the bed and the price before anyone visits. Court approval for the move is your call. The form only asks where it stands, so the home knows the timing.

Find a bed for a client

No patient name on this first request. Homes see the needs, not the person. Names move only after the family or guardian picks a home and agrees.

Discharge documents

Once the family or guardian picks a home, send the 1823, the discharge summary and the medication list by fax straight to that home. Miami Placement sends you the number. A secure upload is coming later.

Please don't email records or attach them here.

How we're paid: homes pay us only when a private pay resident we introduced moves in. Never for a Medicaid resident. Nobody pays to be shown first. I run Mary's House ALF and have a personal interest in it, and I'll say so if it comes up as an option.

Matching homes get the needs and answer yes or no with bed and price. You get the list of yeses.

Can't take a family yourself? Send them to us.

For any facility or office. If a family is a wrong fit for you, too expensive, or you're full, send them our way and they still get help.

Or just text the family's name and number to 305-526-4205, after they say it's okay.

We never pay, reward or trade families for this, and we match every family by fit, not by who sent them.

About hospices

Hospices are Medicare providers. We never pay a hospice or its staff for a referral, and we never take anything from a hospice in return for steering a family or a home toward it.
