All professionals / For rehab centers and skilled nursing

When rehab is ending and they can't go home alone.

The last covered day is coming, therapy has done what it can, and the family knows home won't work. Send me where things stand now, without a name, and I'll find homes that can handle it.

I never pay or reward staff for sending anyone, and the family always chooses the home.

What I know about your day

  • The last covered day comes with little notice.
  • How someone transfers decides more than any diagnosis.
  • Families often hoped rehab would fix everything, and they need time with the news.

How I help

  • Homes answer with the bed and the price, and you see each answer as it comes in.
  • I'm honest about transfers. If they need two people, most assisted living homes can't take them, and I'll say so before anyone tours.
  • I walk the family through private pay and Medicaid long-term care in plain words.
  • You get a one-page summary of every home asked.

Papers the home will usually ask for

  1. AHCA Form 1823, signed by a doctor, APRN or PA. Ask the home how recent it has to be.
  2. PT and OT discharge notes, with how they transfer and walk today.
  3. Current medication list, with the time of the last dose given.
  4. Wound care orders and supplies, if any.
  5. Florida DNRO form (DH 1896), if one is signed.
  6. Equipment they'll need at the home, and when it arrives.
  7. Copies of ID and insurance cards, given to the home after the family picks it.

Every home has its own admission list, and the home decides. Once the family or guardian picks a home, fax these straight to that home. Miami Placement sends you the number. Please don't email records or attach them here.

Suggested form for your work

The same request every professional uses, with the questions that matter in your work added. No patient name, no date of birth, no record number. Homes see the needs, not the person.

How we're paid: homes pay us only when a private pay resident we introduced moves in. Never for a Medicaid resident. Nobody pays to be shown first. I run Mary's House ALF and have a personal interest in it, and I'll say so if it comes up as an option.

Matching homes get the needs and answer yes or no with the bed and the price. You see each answer as it comes in.
