When home has stopped working.

You've seen it coming: the falls, the nights, the caregiver who can't keep up. When the family is ready to look, send me the needs, no name, and we'll find places worth touring.

I never pay or reward anyone for sending a family.

What I know about your day

  • The family usually isn't in a rush, until suddenly they are.
  • You know the person better than any form will.
  • A planned move goes better than a move after a fall.

How I help

  • Homes answer with the bed and the price before anyone tours.
  • If you'd like to be on the tours with the family, tell me and I'll set the time with the home.
  • The family gets a direct number for Miami Placement, so you're not the only one they call.
  • You get a one-page summary of every home asked.

Papers the home will usually ask for

  1. AHCA Form 1823. The home will need one before move-in, signed by a doctor, APRN or PA.
  2. Current medication list.
  3. Home health discharge summary or recent visit notes.
  4. Florida DNRO form (DH 1896), if one is signed.
  5. Power of attorney or health care surrogate designation, if there is one.

Every home has its own admission list, and the home decides. Once the family or guardian picks a home, fax these straight to that home. Miami Placement sends you the number. Please don't email records or attach them here.

Suggested form for your work

The same request every professional uses, with the questions that matter in your work added. No patient name, no date of birth, no record number. Homes see the needs, not the person.

How we're paid: homes pay us only when a private pay resident we introduced moves in. Never for a Medicaid resident. Nobody pays to be shown first. I run Mary's House ALF and have a personal interest in it, and I'll say so if it comes up as an option.

Matching homes get the needs and answer yes or no with the bed and the price. You see each answer as it comes in.
