For guardians
Quarterly visit checklist
If you're a professional guardian, Florida law says you or someone on your professional staff has to visit each ward in person at least once every calendar quarter. This checklist follows what the law says to look at, and turns each visit into a dated report you keep.
Nothing you type here leaves your phone. We don't send it or save it. When you're done, tap Make my report, then print it or save it as a PDF.

What the law says to look at
1. How the ward looks, and their condition
2. Is the current living situation still right?
3. Services: anything new needed, or anything that should stop?
4. Visits and calls with family and friends

Calendar reminders
Add reminders to your own calendar: one at the start of each quarter, one two weeks before it ends, and one a month before the annual plan is due. The law asks for at least one visit a quarter. If you visit more often, pick how often below, and you'll get a reminder for each planned visit too.
This checklist follows Florida Statutes section 744.361(14). It isn't legal advice, and it doesn't replace anything the court asks of you.
