Help

Help for professionals

For discharge planners, case managers, social workers, care managers, guardians and attorneys.

1. Pick your role

  • On For professionals, pick yours. You get a form made for your work and a printable list of the papers homes usually ask for.

2. Send the needs, not the name

  • Fill in Find a bed for a client: care needs, payer, area, room and who signs.
  • Leave out the client's name, date of birth and record number. Homes see the needs, not the person.

3. Homes that fit answer

  • Each home that fits says yes or no, with the open bed and the price it will charge.
  • You get the yeses: each home's license, its letter grade from its AHCA record and the date of that record, and a one-page summary of the homes considered, for your file.

4. The family or guardian picks

  • Once they choose a home and agree, names go to that home, and the papers go straight to it by fax.

5. Harder cases

  • Some needs, like a feeding tube, being bedbound, or nursing around the clock, mean we talk first. Most assisted living homes can't take them, and Florida rules limit it. The home always decides after its own assessment.

6. Guardians and attorneys

  • Each home's page shows whether it works with court-appointed guardians. Court approval for a move is your call; the form only asks where it stands, so the home knows the timing.

7. Look for yourself
